Think through your experience, from the time you saw an ad or made a call, to an appointment or follow-up conversation. No part of the process should discriminate based on race, color, religion, sex (including sexual orientation and gender identity), familial status, national origin, or disability. Additionally, think about whether you know of other people who had a different experience than you did?

Record

Write down names, addresses, and phone numbers of individuals, agents, properties, companies and witnesses involved, as well as of any person who may have had a different experience with the same individual or company. Note dates, times, and comments from conversations or other incidents. Keep copies of advertising, letters, emails, and any other documents or information.

After you submit the form, we will contact you to learn more, help you identify and further document potential discrimination, and take steps to investigate the situation.
